There are a lot of implications... ex: AI must be able to fight 2 opponents. The player has to be fully re-animated from 3rd person (the animations look nice in 1st person but horrendous in 3rd person). Netcode... etc...
Reanimating all the models in 3rd person sounds like it would be difficult with the variety of attacks already in the game.
The only team that have been able to pull off a first-person melee game in Source (effectively) is the
Pirates, Vikings, and Knights mod team. Although Zeno Clash is paced faster and would most likely be difficult to replicate something as accurate.
I'm indifferent of the inclusion of some form of multiplayer in Zeno Clash 2. I think it would help Sales sure, but I don't think the ends justify the means (too much development time, funding, etc.). As Carlos pointed out the Netcode on the Source engine would make the expericence frustrating, if they switch engines that might mitigate some of the issues. I have always thought Zeno Clash was envisioned to be a single player experience, and changing that at this point would only detract from Zeno Clash 2 (which may or may not have multiplayer).
